On-Site Assessment of Your Loved One’s Needs
First, we’ll ask you to tell us about your concerns for your family member. Then, a care manager will visit and conduct a thorough on-site assessment to determine the current and future needs necessary to enhance your loved one’s life. In this initial visit, a care manager will assess health, functional, psychosocial, and safety needs.

  • Does your family member remember to take prescription medications properly?
  • Does he or she socialize? If so, how often?
  • Is the house clean?
  • Are all the appliances, doors, and window locks in working order?
  • Is there fresh food in the refrigerator?

Our care managers know how to get to the relevant issues as sensitively and unobtrusively as possible. Following the assessment, we’ll provide an in-depth analysis of your family member’s current status. This is accompanied by a detailed care plan with recommendations for cost-effective services and resources. All information and recommendations are strictly confidential.
